St. Josmane's Hope

St. Josmane's Hope was the Cadian System's penal world, a continent-sized military prison, not a fortress-kasr. Prison riots handed it to Chaos early in the 13th Black Crusade, and Creed ordered Strike Force Herald to overload the generatoria rather than let it become a staging world. It died before Cadia itself cracked.
