Old One Eye is a Carnifex that should have stayed dead on Catachan and did not. A regiment killed it. The jungle kept the corpse. Years later it got up with a hole where an eye had been and went back to work as if death were a rest period.
It has been sighted on other worlds, including Tarsis Ultra, which is how a single bioform becomes a legend the Munitorum has to file under more than one war. Hive fleets do not usually do individuals. This one does, and the scar is how the Guard know they are looking at the same monster.
The Imperium has killed it on paper more than once. The paper is not the problem.
